I am the parent/legal guardian of the participant(s) listed, and hereby grant my permission for him/her/them to participate fully in Frolic. I give my permission to: Release this information to the church office, include my child(ren)'s likeness on the church's website, facebook page, etc. I understand that my child(ren)'s name(s) may be used in conjunction with the likeness. In the event of an emergency, and I cannot be reached, I give permission for supervising Trinity staff or the available adult leader to sign forms that would ensure the necessary and immediate treatment of my child(ren). I understand that my typed full name and date is my legally binding signature in terms of this agreement.
